Anytime you travel for a fieldtrip with Bellevue College you need to complete the following forms. If you are under 18 at the time of travel, you will be required to complete a different set of forms that you can obtain from Kyle Barber (kyle.barber@bellevuecollege.edu).

Please download, print, and fill out each of the following forms. Return submitted forms to Kyle Barber or one of the DECA officers.

• Release of Liability and Medical Release
